A Reflection from The Psychiatric Nurse Practitioner’s Handbook
One of the most important questions in psychiatric practice is also one of the simplest:
What happened before this?
A patient becomes agitated, someone stops coming to appointments, a person who had been doing well suddenly begins using substances again, or a patient refuses medication.
It is tempting to focus on the behavior in front of us, yet behavior rarely begins at the moment we notice it. Something came before. Perhaps it is a loss, a conflict, a fear, or a memory. Perhaps it is a change in routine, a feeling of being misunderstood, a side effect no one asked about, or a hope that quietly disappeared.
The behavior may be the most visible part of the story, but it is not necessarily the beginning of the story.
This is why good psychiatric care requires more than asking, “What is happening?”
We also ask: “What happened before this?”
That question moves us from reaction to formulation. It helps us identify patterns, understand triggers, recognize unmet needs, and discover opportunities for prevention.
The Psychiatric Nurse Practitioner who understands what came before is better positioned to influence what comes next. When we understand the sequence, we gain something more powerful than an explanation. We gain the possibility of prediction; and prediction gives us the opportunity to intervene earlier.
More to come.
